Transcripts from ALL institutions, previously attended and currently attending (including the University of Utah) regardless of final grades earned or how long ago attended. DO NOT PAY TO SEND YOUR TRANSCRIPTS TO THE ADMISSIONS OFFICE. You will need to upload either the Official or Unofficial copies to your BSW Program Application.
Resume
Resume outlining volunteer and/or professional work experience related to human service/social work. Resume must be typed, no longer than two pages, in reverse chronological order, and attached as a PDF. For each experience, include your position/title, the name of the organization, beginning and end dates, average weekly hours worked AND total number of hours worked, and a detailed description of your responsibilities or the service provided.
Two Recommendations
Recommendations should come from either working professionals in the human service field, or individuals who can speak to your potential in the BSW Program based on your academic/professional performance. Recommendations are requested in a separate form from your application and should be requested AT LEAST 4 weeks prior to the submission deadline. Recommendations from personal counselors, religious leaders, or family members/friends in social work will not be accepted.
Personal Statement Essay Questions
