CSW Ph.D. PROGRAM STUDENT FORMS

Approval for Credit Memo: This memo should be submitted to the Ph.D. Director by students asking for approval of credit toward the outside elective requirements or for approval of credit hours not used toward another degree when requesting a waiver.

Incomplete Grade Contract: This contract should be completed by the student with the instructor, then forwarded for retention in the Ph.D. Student File whenever an Incomplete grade has been given and the work will not be completed by the end of the next semester.

Independent Study Contract (for SW 7900): This contract should be completed by students who wish to arrange independent study hours with an instructor. The instructor of record for all SW 7900 hours is always the Ph.D. Director; this form is used to submit a grade for the course.

Off Site Presentation Request: This form should be used when requesting the services of the Eccles Broadcast Center as the origination site for dissertation proposal and defense presentations. In most cases, the costs incurred for the EBC equipment and charged to the CSW Ph.D. Program should be reimbursed by the student.

Personal Plan of Study: After 18 hours of CSW Ph.D. core classes are completed, students select a pre-candidacy advisor. This form should be completed by the student with the help of the pre-candidacy advisor and submitted to the Ph.D. Director for approval.

Proposal Approval: Student should complete this form which is signed by the Dissertation Supervisory Committee the day of the proposal presentation and forwarded for retention in the Ph.D. Student File.

Supervisory Committee Request: Upon satisfactory completion of the QE and advancement to candidacy, a student should complete this form to request approval of a dissertation committee. Changes to the Committee also have to be approved by The Graduate School.

Leave of Absence: A student seeking a leave must have completed a minimum of one semester (9 hours) with a GPA of 3.0 or above and should submit a written request to the Ph.D. Director along with The Graduate School form.

Program of Study: Students are required to complete and file this form with the Graduate Records Office at least one semester prior to the planned dissertation defense and/or graduation. This form is usually signed the day of the defense proposal presentation. All required course work including the allied outside elective hours should be listed. Incomplete grades will not be accepted with the exception of the “T” grade for all dissertation research hours

Report of the Final Oral Examination: This form is prepared by the student and completed by the Dissertation Supervisory Committee, then forwarded to the Ph.D. Secretary for processing the day of the student’s defense presentation. The other forms required at this time are prepared on the thesis paper and are usually prepared by the student’s dissertation editor.

Graduate Travel Award: The Graduate School may provide a travel award to Ph.D. students. The College of Social Work may provide matching funds with a maximum of $500. The application should be sent to The Graduate School with a letter of support from the Ph.D. Director.
