Application Checklist
Application materials are due February 1st for campus classes. Newly admitted campus students begin classes only during the fall semester. We are not accepting new TED applications at this time.

University of Utah)

2. Personal statement of intent describing professional interests and career aspirations, reasons to receive advanced
research training in social work, major areas of interest in research or social work practice, practice experience in
social work, plans regarding financing a Ph.D. program, and post-Ph.D. plans.
3. GRE scores (test taken within the last 5 years)
4. Sample of paper: published, submitted for publication, or prepared for application
5. A Letter of Reference from at least three people who can attest to
your personal qualifications, character, and dependability. These letters should be submitted directly to the College
by the writers and forwarded in a signed and sealed envelope.
Please advise the University and CSW of any changes to your contact information or your academic plans after the application has been filed. It is the applicant’s responsibility to see that the application is completed. Recommendation for admission will occur only after the application is complete and has been reviewed by the Ph.D. Admissions Committee.
